How to Fill Out USPS Form 1583
A Step-By-Step Guide
What is it?
USPS Form 1583 authorizes a Commercial Mail Receiving Agent (CMRA) like The Pioneer Collective to receive mail on your behalf. If you're setting up a mailbox service with TPC completing Form 1583 is essential to ensure the USPS recognizes your agent as authorized to handle your mail.

Part 1: Required Documents
Before filling out Form 1583, gather two forms of identification. At least one must contain a photo. Here are some acceptable forms of ID:
Acceptable Photo ID Options: Driver's license, state ID, passport, armed forces ID, university ID, recognized corporate ID.
Acceptable Address / Non-Photo ID Options: Current lease, mortgage, deed of trust, voter registration card, vehicle registration, driver’s license, home or vehicle insurance policy.
Note: Social Security cards, credit cards, and birth certificates are not acceptable forms of ID.
Part 2: Filling Out Form 1583
Box 1: Enter today’s date.
Box 2: Write the name of your business or organization if you’re setting this up for a company. If you’re an individual, leave this box blank.
Box 3: Write the full mailing address of your CMRA, including any specific suite or mailbox number they provide to you. This will be the address where your mail is delivered.
Box 4: Enter the name and address of the CMRA company that will receive your mail. If you downloaded Form 1583 directly from a CMRA’s website, this section may already be filled in.
Box 5: Sign this box if you want the CMRA to receive restricted delivery mail on your behalf. Restricted delivery includes packages or mail requiring special handling.
Box 6: Enter your full legal name (or the name of the applicant applying for mailbox services).
Boxes 7a-7e: Provide your home address, phone number, and email. This must be a U.S. residential address where you can be reached.
Box 8: List two types of identification and their associated numbers. If you complete this form in front of the CMRA staff, they can fill out this information for you. The CMRA will also need to verify these IDs.
Box 9: For businesses, enter the business name here. If this mailbox is for personal use, leave this box blank.
Boxes 10a-10e: Provide the business’s permanent address and phone number. This cannot be the CMRA’s address and should match the business registration or a home address if the business is not incorporated.
Box 11: Describe the type of business (e.g., “Marketing,” “Consulting,” “E-commerce”).
Box 12: List any additional individuals authorized to receive mail on behalf of your business.
Box 13: If you’re a corporation, list the names of the corporate officers (e.g., CEO, President).
Box 14: If your business is registered, include the county, state, and date of registration.
Box 15: Sign and date to authorize the CMRA to handle mail on your behalf. If you’re not signing this form in front of CMRA staff, the form must be notarized.
Part 3: Returning Form 1583
Once you've completed the form:
In Person: If you’re able to visit your chosen Pioneer Collective location in person, bring your completed form and two forms of ID. Sign the form in front of TPC staff to avoid needing notarization.
By Mail or Online Notarization: If you can’t visit in person, you can have the form notarized in one of two ways:
At a Notary Public: Take the completed Form 1583 and your two IDs to a notary. The notary will:
Verify your identification.
Fill in Box 8 with ID details.
Sign Box 15 or provide certification on a separate sheet.
With an Online Notary: Many online notary services are available that allow you to complete the notarization remotely. After completing the online notarization, you’ll receive a notarized copy of the form. Ensure the online notary you choose meets USPS requirements.
Email or mail the notarized form to your TPC along with copies of your identification documents.
